1. Do you accept insurance?
Prime Weight Loss LLC is a fee-for-service practice and does not participate with insurance plans, Medicare or Medicaid. However, I can provide you with a superbill which you can submit to your insurance company for a reimbursement.

2. What forms of payment are accepted?

Cheque, Cash and Apple pay.

3. What is your cancellation policy?
Please cancel any appointments within a 4 hour notice, otherwise you will be charged the full fee for no shows or late cancellations.

4. What should I expect during an initial evaluation?
Initial evaluations typically take about 90 minutes, where we will discuss and review history and concerns. After the initial evaluation, I will make recommendations for the treatment plan. Follow up appointments can typically run 45 minutes to an hour long.

5. What age group can be treated?
I can treat age 18 and up.

6. What is your emergency policy?
In case of an emergency, please call 911 or go to nearest emergency room.
